Student Agreement or Affidavit
(Student statement that "I am ready to begin.")

Patrick Henry Community College Logo

When you’re ready to begin this course and can accept your responsibilities mentioned above, send a copy of the following affidavit to the instructor:

Dear Professor Meyer,

1.  I am a registered student in Survey of Economics.
2.  I’ve gotten my required text and study guide.
3.  I can see that the Syllabus is divided into menus called "Getting Started, Need-to-know, Nice-to-know, and For your information.

(I know that I should use Internet Explorer because Netscape Navigator won't work as well and enter Survey of Economics either through BlackBoard Portal or http://fpwww.ph.vccs.edu/eco  , the instructor homepage.)

4.  I’m ready and able to navigate the links in these menus, and can understand each part of the Syllabus and 15-Module Calendar by using a school or personal computer with Internet Explorer.

5.  I can follow the directions in the How-to-Prepare Link when the 15-Module Calendar says that it is time for Exams, Electronic Participation, and Essays.  I also know there is a great deal of electronic assistance stored on MyEconLab, including tutorial assistance of up to three calls per evening.

6.  I'm willing to contact my classmates when they or I need a study partner electronically. I know that Patrick Henry Community College (PHCC) webmaster - Mark Nelson, (276)656-0275, or mnelson@ph.vccs.edu, can provide tutorials to help me with Discussion Board, student email, and with access to materials stored on BlackBoard at PHCC.  I can ask for/complete, and return a Proctor Request Form from Ms. Carol Ford, cford@ph.vccs.edu if I plan to take my exams at a location other than Patrick Henry Community College.

I will introduce myself to my classmates and look forward to working independently or with study partners on the electronic and hard copy materials in Survey of Economics!

Sincerely,
